1) Go to: TeenChoicesandChallenges.com

2) Click on "Sign-Up" and then click on "click here" (the first link on the next page) and then click on the "I am a member" button if you are a Kaiser Permanente member.

3) Fill out the online questionnaire. Your answers will be kept private and confidential.

4) Read the personalized material after you finish the questionnaire. The computer reviews your answers and creates a set of information to help you deal with whatever is going on in your life, including phone numbers of who to call if you need more help.
