Industrial injury (Worker's compensation)

An industrial injury is defined as any injury suffered in the course of your work or at your workplace resulting in a temporary incapacity, subject to medical evidence.

When injuries occured, please follow the following steps for filing a worker's compensation (work injuries, occupational injuries):

  • Notify your employer that you are injuried.
  • Go to the designated health provider or nearest Emergency Services.
  • Complete this form prior to receiving treatment--Industrial Injury Form.
  • Notify the health provider if this is a follow-up appointment from work's compensation.
