What is the cost of LASIK?

SERVICES & PAYMENT OPTIONS

Services Offered by Kaiser Permanente:

  • LASIK - $1,900.00 per eye
  • PRK - $1,900.00 per eye
  • Wavefront LASIK - $2,400.00 per eye
  • Pre-Operative Evaluation ONLY - $50.00. This fee is waived if surgery is subsequently performed by us within 90 days.
  • Pre- and Post-Operative Care ONLY (Co-management) - $400.00 per eye
  • Second Opinion - fee to be determined when booking appointment

LASIK and PRK Procedure Includes the following:

  • Pre-operative evaluation including corneal topography, pachymetry, dilated and non-dilated refractions, eye pressure, and assessment of Laser Vision Correction candidacy
  • Professional Surgical Fee
  • Laser Center Fee
  • Vision Key Card (Royalty Charged by Laser Manufacturer)
  • Twelve (12) months of post-operative care
  • Twelve (12) months of coverage through Kaiser Permanente Ophthalmology and Optometry Services for professional fees on any re-treatment procedure (and associated care)
  • Post-operative surgical pack with prescription eye drops

The procedure fee DOES NOT include the following items:

  • Any unscheduled visit or care for unrelated eye conditions
  • Glasses for reading or night vision
  • Outside second opinion

METHODS OF PAYMENT

Kaiser Permanente is pleased to offer a patient payment plan. The patient payment plan is a quick and easy way to make Laser Vision Correction affordable on a monthly basis. Credit lines are available with interest rates as low as 9.9% with terms varying from 12 to 48 months. There is also a No Interest Payment Plan available for 3 and 6 month terms.

Use your employer-provided Flexible Spending Account (FSA) or Medical Savings Plan (MSP) to help pay for Laser Vision Correction. Save as much as $2,400, depending on your income and tax bracket. Consult your employer’s health plan administrator for details about FSA or MSPs.

Payments may be made by cash, check, credit card (Visa and Mastercard), or money order (payable to Kaiser Permanente). Bankcards are accepted at select Kaiser Permanente locations.

Payment for surgery must be received one week prior to your procedure.

CANCELLATION & REFUND POLICY

For cancellations made more than one week prior to the scheduled surgery date, the payment will be refunded in full.

Cancellations made within one week of scheduled surgery date are subject to a 10% cancellation fee. This cancellation fee will be waived if the procedure is re-scheduled and performed within 90 days.

If the surgeon cancels or if there is a clinical reason for the cancellation, no fee will be incurred.

No portion of the fee will be refunded after surgery is performed.
