The Department of Medicine provides comprehensive, coordinated primary care, and expert subspecialty consultations for adults 18 years and older. We have a long-standing tradition of excellence in primary care, and serve a culturally and socioeconomically diverse patient population in the Bay area.

The Department of Medicine in San Francisco provides adult primary care services with a team of professionals including your Primary Care Physician or Nurse Practitioner. The department is organized according to the Adult Primary Care (APC) model of health care. The APC model, an innovative system of providing individualized medical care in the outpatient setting, consists of physicians and nurse practitioners working in concert with allied health care professionals to provide a multidisciplinary approach to patient care. As primary care providers, we place a strong emphasis on preventive care, health education, and appropriate disease management and medical interventions. Your relationship with your Primary care provider is designed to be long term and ongoing, in order to provide continuity of medical care and allow you to make informed decisions about your healthcare needs.

We have five APC modules each consisting of internists, nurse practitioners, a Diabetes nurse case manager, a clinical health educator, a behavioral medicine specialist, a physical therapist, a nurse case manager who assists with the care of patients with complicated complex medical conditions, and medical assistants. Three of these modules also serve specific needs of our members: members who prefer to speak in Cantonese or Mandarin, who prefer to speak in Spanish, and who seek special expertise in HIV care. The subspecialty services of Oncology, Hematology, Nephrology, Gastroenterology, Pulmonology, Rheumatology, Infectious Diseases, Endocrinology, Travel Medicine, and Community Based Medicine are also part of the Department of Medicine, work closely with your primary care provider in a unique group practice, and are accessed through referrals from your primary care provider.

In addition, the Department of Medicine utilizes Chronic Disease Management Programs to provide comprehensive care to our members with chronic health conditions. Programs include Asthma, Cholesterol, Diabetes, and Heart Failure Management, Complex Chronic Conditions Case Management, and Multifit- a program for patients with cardiac conditions. These programs are accessed through referrals from your primary care provider.
