PHASE: Prevent Heart Attacks and Strokes Everyday

When you have heart disease or diabetes you have an increased risk of heart attack and stroke. That's also true if you've had a previous stroke, chronic kidney disease, peripheral arterial disease, or an abdominal aortic aneurysm.

The good news is there are steps you can take to help reduce your risk of heart attack and stroke:
