Su número de expediente médico

Usted recibirá una tarjeta de identificación de Kaiser Permanente con su número de expediente médico. Si no la recibe antes de su primera cita, puede ponerse en contacto con el Departamento de Servicios a los Miembros llamando al 1800-788-0616, los siete días de la semana, de 7 a.m. a 7 p.m.; los usuarios del teléfono de texto TTY (para personas con problemas auditivos o del habla) pueden llamar al 1-800-777-1370.
