Welcome to the Kaiser Permanente Oakland Continuing Care website! Our team includes physicians, nurse practitioners and patient care coordinators. We are a diverse group that reflects the population of our service area. We work together to care for our Kaiser members in 34 skilled nursing facilities in Oakland, Alameda, Berkeley, San Pablo, San Leandro and El Sobrante. Patient care coordinators work with patients & their families to help patients make the transition from hospital to skilled nursing facility & from skilled nursing facility to home. Continuing Care physicians & nurse practitioners make regular visits to the skilled nursing facilities to provide medical care for our members who are admitted for rehabilitation after surgery or a medical illness. We also provide medical care for our long-term residents in these facilities. Our goal is to promote & maintain the health of all our patients in skilled nursing facilities & to assist them to regain the highest level of function. We also work closely with Hospice when a patient is receiving end of life care. We look forward to working with you.
