Our job is to promote quality health care and services to all Kaiser Permanente members.

In order to provide you with prompt, quality, and accurate services, we provide forms that can assist you in a timely fashion.

Protect patient's privacy and confidentiality is our priority, and therefore, before we release any medical information, an authorization form needs to be signed by the patient or the pre-assigned power of attroney. Click here for the form Patient's authorization form.

Please complete this form, sign and date before visiting the Business Office.

If you are entitled to receive a refund, please complete the refund from. Click here for the Refund Request Form. After completing the form, you may fax it to the Business Office. If you need any adjustment to a bill you received, please have the account number, Kaiser medical number, and date of service ready before contacting the Business Office.
