Personal Practitioners

An important part of your health care experience is building a relationship with your personal physician or nurse practitioner. To that end, we encourage all our patients to have a personal practitioner who practices at a location convenient to your home or office. To select a personal physician or nurse practitioner at Kaiser Permanente San Jose or Gilroy facilities, contact one of our Member Outreach Coordinators at (408) 972-7321 or toll-free at (877) 327-3663. You may also select a personal physician on our website: www.kpsanjose.org or www.kpgilroy.org.

Please understand that some physicians and nurse practitioners may not be accepting new members at the time you make a selection. You may need to make another choice. If for any reason you would like to change your selection, we can assist you.
