About Your Office Visit

Prevention Information on Your Registration Slip

Welcome to my home page! I am located in the Internal Medicine Department #260 at Kaiser, Santa Clara on the Homestead campus. I am usually in the office Mondays thru Thursdays. I see patients in the clinic and occasionally teach medical residents. I can speak Cantonese, a Chinese dialect. I've included a few links that I hope you find useful. If you have any questions please call the numbers listed above. My personal medical assistant is Sarah. Our charge nurse is Marva.

About Me:

Dr. Fong is a native Californian who grew up in Sacramento. She attended the University of California, Davis where she majored in biological sciences. She then received her medical degree from the University of Southern California and completed her internal medicine residency at the University of California, Irvine. She is board certified in internal medicine.

Dr. Fong joined The Permanente Medical Group in 1996. She is currently a member of the teaching staff for the Kaiser Santa Clara internal medicine residency program and is a part-time preceptor for the San Jose State University Nurse Practitioner Program. She is also a member of both the American College of Physicians and the American Medical Association.

In addition, Dr. Fong is fluent in Cantonese. In her spare time, she enjoys playing the piano, gardening, and spending time with her husband, son and daughter.

In the office, Dr. Fong is supported by her personal medical assistant, Sarah and nurse Marva. If you forget to ask a question during your visit, you may leave a message for either Dr. Fong or Sarah at (408) 554-9800. Please be sure to include your medical record number, name, and time of day that you can be available to accept a return call from either Dr. Fong or Sarah. For prescription refills, it is recommended to call the pharmacy refill phone line at (408) 236-5690. Since Dr. Fong and her medical staff are busy attending to patients who have scheduled appointments at the clinic, it is requested that you call to leave a message; unexpected walk-ins are not encouraged. Thank you.

If you think you have a MEDICAL OR PSYCHIATRIC EMERGENCY, CALL 911 IMMEDIATELY or go to the nearest hospital. DO NOT attempt to access emergency care through this web site. An emergency medical condition is a medical or psychiatric condition that manifests itself by acute symptoms of sufficient severity (including severe pain) such that you could reasonably expect the absence of immediate medical attention to result in any of the following: serious jeopardy to your health, serious impairment to your bodily functions, or serious dysfunction of any bodily organ or part. An emergency medical condition is also "active labor," which means a labor when there is inadequate time for safe transfer to a Plan hospital (or designated hospital) before delivery or if a transfer poses a threat to the health of the member or unborn child.
