- If you have an in-person appointment, the Sleep Medicine Physician will write the prescriptions and submit the various referrals. If you choose a telephone appointment, the referring provider will write the prescription and submit the various referrals. Follow-up care after the initial in-person or telephone appointment will be conducted by the referring provider or Primary Care Physician.
