As part of your initial visit with the provider, you are asked to complete paperwork. Please download the necessary forms and complete the information as concisely as possible. Bring the completed paperwork with you 15 minutes prior to your scheduled appointment.
NOTE: Different forms are needed for different types of appointments. Failure to bring completed paperwork may result in your appointment being delayed or cancelled.
- If the appointment is scheduled for an adult, you will need to complete the form for an adult visit. Example: "Adult Personal Data Sheet"
- If the appointment is scheduled for a child, you will need to complete the form for the parent/guardian. NOTE: If your child is 12 years or older, there is a form he/she must complete as well. Example: "Child, Adolescent, & Family Data Sheet" and "Adolescent Questionnaire" (if 12 years old or older)
- If the appointment is scheduled for an adult or adolescent for a Chemical Dependency matter, you will need to complete either the adult or adolescent forms. Example: "Adult Personal Data Sheet", "Adolescent Personal Data Sheet" (if 12 years or older) and "Parent Data Sheet for Adolescent Program"
