Communicate Your Health Care Wishes

If an illness or accident left you unable to make decisions for yourself about things that really matter to you:

  • Who would make those decisions for you?
  • How would they know what you would or wouldn't prefer?

Give your loved ones peace of mind; make your wishes known NOW while you are capable. Take the time to discuss with your family, friends and physicians about your wishes for medical care if you were to become seriously ill.

In addition to talking with your loved ones, you can let your wishes be known through an Advance Health Care Directive. An Advance Health Care Directive is a legal document that tells your physician, your family members and friends about what kind of care you would like to have if you become unable to make medical decisions. It's called an advance directive because you choose your medical care before you become seriously ill.
