• If you want to begin treatment for an on-the-job injury with Kaiser, please report the injury or condition to your employer and request to be treated here. Ask your employer to call us at (408)972-6800 for a same day appointment. (Note: Your employer may in some cases have the right to direct you to a doctor of their choice for care instead.)

• If you are already under care elsewhere for an on-the-job injury, and your are considering transferring your care to Kaiser Permanente, call your Workers’ Compensation Insurance claim administrator to discuss your reasons and to request a transfer. If you have questions or need help, please call us at (408)972-6333.

• If you need an employer-paid physical for your work, ask your employer to call (408)972-6800 to schedule it with us. If you are paying for your own DMV physical please contact us yourself at the above number for an appointment.

Note: You are welcome to make use of our Occupational Health services whether or not you are a Kaiser Health Plan member.
