• The Admitting Department is located on the first floor lobby of the main entrance of the hospital.
• Hours: Monday through Friday 6.00 AM to 10.00PM

Saturday and Sunday 8.00 AM to 10.00PM

• You will receive a pre-admission form at your 30-32 week prenatal appointment.
• If you are a Hayward patient, you may complete your form and take it to the Admitting Department.
• In Fremont, the Ob/Gyn Department will forward your completed form to the Hayward Admitting Department.
