A personalized approach to Healthier Living and a Healthier Heart.

If you have had a heart attack or bypass surgery in the last month, you may want to join the Multifit program. Multifit can help you feel better and help you take charge of your heart health. The program is called Multifit because it can help you make lifestyle changes to reduce multiple heart health risk factors so you become fit. Multifit was designed and researched by Stanford's Cardiac Rehabilitation Program and Kaiser Permanente.

When you participate in the Multifit program, you have a cardiac nurse as a personal care manager for up to one year. Your nurse will provide guidance and information to help you make the lifestyle changes you choose. To make the program easy for you, we have designed it to be home-based. You will meet with your nurse by telephone and progress reports will be sent to you at home. While you are in Multifit, you can also continue to see your primary care physician for medical and follow-up care.

Multifit focuses on:
• Exercise
• Stopping smoking
• Low-fat diet
• Cholesterol management

Much of your rehabilitation can take place at home with the support of your family and friends. In addition to Multifit, you may be interested in additional resources for support and for learning about how to reduce your coronary risk factors.
