If you are entitled to receive a refund, please complete the refund from. Click here for the Refund Request Form. After completing the form, you may fax it to the Business Office. If you need any adjustment to a bill you received, please have the account number, Kaiser medical number, and date of service ready before contacting the Business Office.

If you would like to request a refund from the pharmacy, please go to the pharmacy where you have obtained medicine.
