Express Your Wishes: The Advance Directives Class
An advance directive lets you give instructions about future medical care, explain your wishes, and designate someone you trust to make decisions for you in the event you’re unable to speak for yourself. Forms are provided, along with guidance on how to use them. Open to the community.
~ Location: Fremont and Hayward Medical Centers
~ Dates/Time: Offered quarterly. One 2-hour session; call for dates and times
~ Fee: No charge
