Patients are strongly encouraged to have a Medical Advance Directive on file with Kaiser Permanente. These help us to know your wishes should you be incapable of making medical decisions on your own for any reason.

These forms are available at Kaiser Permanente Health Education Centers at no charge, or they can be ordered from the California Medical Association for $5 at CMA Advance Directive.

After you complete the forms, bring them to the business office of your Kaiser Permanente facility and they will be recorded and distributed.
