Some woman will suffer what is called menstrual migraine, which is a migraine headache that occurs in and around the menstrual cycle. In this situation most of the medications that have been mentioned for migraine can also potentially be effective. Additionally using a nonsteroidal anti-inflammatory medicines such as naproxen, up to 500 mg bid, starting a day or two before anticipated headache and taking it for several days can be helpful. There are some women that have tried magnesium supplementation and reported benefit. Lastly, if none of the standard migraine treatments or the others just mentioned are helpful, there are various hormonal manipulations that have been tried with success and again you will need to discuss this with your physician. The other issue for women in migraine is during pregnancy. The safest approach in pregnancy regarding migraine is to not be on any medications. Therefore non-drug therapies such as relaxation techniques, adequate sleep and even massage, ice packs and biofeedback have all been tried with some benefit. The triptan drugs such as Imitrex, Maxalt and ergot drugs should never be used at all. Acetaminophen can be used. If you absolutely need another medication, Compazine suppositories for both nausea and vomiting as well as migraine can be used as well as narcotics. If a patient suffers from severe migraines during pregnancy and needs to be on an ongoing medicine in order to prevent them, a physician would have to review the risks and benefit with the patient and sometimes a medication such as propanolol can be used.
