Not all health plans provide the same coverage for fertility services. We advise you to check your specific benefits with member services. We also strongly encourage you to attend our Fertilty Class. This is a two hour presentation to introduce you to the complex area of human fertility, anatomy, physiology, common problems and basic laboratory testing. Our manual, The Art of Conception is included in its entirety here for your convenience.
You can register 15 minutes early to complete the questionnaire or can download the fertility questionnaire and complete it at home and bring to your appointment.
