For your appointment with the gyn clinic

• Please remember to bring your Kaiser I.D. card and a photo I.D. and your co-pay if applicable.
• If you have missed a period and want to know if you are pregnant, please go directly to our lab and ask for a urine or blood pregnancy test. You may call 510-675-4090 for Hayward or 510-248-3040 for Fremont on the following day for your results.
• We prefer if you abstain from sex, douching, or inserting any vaginal creams for 24-48 hours prior to your appointment. Also it is helpful if you bring your medications or any supplements and creams you are using with you to your appointment.
• If you are a new patient or having a routine Gyn visit, please register 15 minutes early to fill out the health questionnaire. If you choose, you may download the questionnaire by clicking here Ob/Gyn Health Questionnaire. Complete the form at home and bring it with you to your appointment. This will save time and allow more time with your doctor or nurse practitioner.
