In order to schedule a physical or occupational therapy appointment you must have a referral from your primary care provider. 24 hours after this referral has been made please call the facility where you wish to receive your care (Fremont or Union City).

Physical therapists are available for consultation in adult medicine. You may request to see a physical therapist if you feel you have a problem that involves your spine, joints and /or muscles by calling (510)675-4010.

If you are hospitalized and physical or occupational therapy services are requested by a physician these services will be provided by a therapist in the hospital.
