My medications have changed since I was discharged, how do I know if they are correct?

Often times, when you are discharged from the hospital, you will have new medications or new doses of your old medications. These changes are based on your response to medications while in the hospital and sometimes on laboratory data that we reviewed while you were in the hospital. Some medications need to be adjusted after you go home because you are on a different diet and under different levels of stress at home.

To help you keep track of your medications, keep a record every pill you take. One good way to do this is to write down your daily schedule of medications - what you take and when you take it. When you go in to see your personal physician, bring this schedule with you as well as your medications so that your personal physician can see the medicines you've been taking and how frequently you've been taking them. Then she or he can make changes as appropriate.
