Adult Asthma Program

Welcome to the home page for the Adult Care Management Program. This site is designed to help you become familiar with our program. We also want to provide you with links to other related resources such as "asthma tips", pharmacy, the Health Encyclopedia and more.

The Adult Asthma Program was created to help you better control your asthma. This will mean better quality of life for you and your family.

The Adult Asthma Program will help you achieve your goal of better asthma control in many ways. Once enrolled, you will have face to face visits with one of our care managers, intensive education, medication adjustments and trigger controls. Follow up will include telephone appointments and return visits as needed.

Our care managers are Registered and Licensed Respiratory Care Practitioners or a Registered Nurse with special training and skills on state of the art adult asthma care. These practitioners work under close supervision of a physician.
