OUR DEPARTMENT

Welcome to the Social Services Department. We offer social services and discharge planning to patients of all ages and their loved ones. Our services are available to health plan members without a physician’s referral.

OUR TEAM

Our skilled and caring team includes Clinical Social Workers and Patient Care Coordinators. Our staff is unit-based in the hospital. We work collaboratively, as part of hospital-based teams, with physicians, nurses, physical therapists, dieticians, and therapists.

HOW TO GET CARE

Referrals are accepted from physicians and other health care professionals, as well as directly from patients, families, and community providers.
