Message from the Chief of Adult Medicine
|Physicians and Nurse Practitioners || Adult medicine physicians and nurse practitioners provide general health care for acute and chronic diseases as well as preventative maintenance. This link takes you to the list of our providers|
|Physical Therapy|| Physical therapy provides a full set of rehabilitation services. This link takes you to details of our outpatient rehabilitation services, location and hours, therapist biographies, and educational information links|
|Adult Medicine Pharmacists|| Provides appointments to help patients find the lowest cost medicines, to manage their blood pressure, cholesterol, low risk diabetes, thyroid and other conditions, provide medication management program for patient who get their medicines under Medicare Part D, and provides general information about medicines the patients are taking|
|Behavioral Medicine|| Provides basic counseling services for problems such as stress, depression, anxiety, grief, etc.|
|Chronic Condition Nurse || Provides individual one-on-one assistance for patients with multiple chronic diseases who need additional help managing there medicines, understanding their diseases, and assistance with basic living needs. Please ask your primary care doctor for a referral|
|Clinical Health Educators || Provides classes and individual appointments to help patients loss weight, quit smoking, reduce stress, manage chronic conditions and provides HIV counseling|
|Asthma/COPD Care Management Program || The Asthma/COPD Care Management Program provides education, assessment, and management for patients with asthma and/or COPD. Please ask your primary care doctor for a referral|
|Registered Dietitian || Provides both classes and individual education on diets including diabetes and cholesterol diets. Please ask your primary care doctor for a referral|
|Cardiovascular Risk Care Managers|| Provide specific services for high risk patients for hypertension, diabetes, elevated cholesterol especially for the patients that standard therapies have not been effective. Please ask your primary care doctor for a referral|
