Welcome to the Anticoagulation Service at the Santa Clara Medical Center.

The Anticoagulation Service is also known as Warfarin or Coumadin Clinic.

We are located at our new medical office building on the corner of Lawrence Expressway and Homestead Road in Santa Clara, department 460. Our home page is designed to help you get acquainted with the Anticoagulation Service and get access to useful information regarding anticoagulation and other health information.

The Anticoagulation Service is dedicated to help your physician manage your anticoagulation therapy and provide you with comprehensive information regarding anticoagulation medications, mainly warfarin.

You will be working with a panel of 2 main clinical pharmacists depending on your last name. You will also be invited to attend warfarin class upon enrollment.

Medication Safety: Many factors can affect the activities of anticoagulation medications . Please call us at 408-366-4323 to inform us of any changes in your medications, diet, or your health condition since these changes may alter your anticoagulation blood test results. It is for your safety that we ask you to call us when you have the above changes.
