ADHD Rating Scales for Parents & Teachers

Diagnosis and treatment of ADHD requires information from parents and teachers about a patient's behavior at home and school. In order to assess your child for ADHD and do follow-up checks, parents and teachers should complete the ADHD rating scale forms below. The ASSESSMENT form is for the initial screening for ADHD and other conditions. Subsequently, the FOLLOW-UP forms should be used. If you have questions, ask your physician.
