Sleep Apnea Program

Dear Kaiser Patient, Your primary care physician has referred you to the Pulmonary Department at San Jose to test your breathing while you sleep. You and several other patients with similar problems will receive instructions on how to use the equipment in a one-hour appointment with a Respiratory Therapist. The test will require you to wear the equipment overnight while you sleep, and return it the next day to the Pulmonary department. Please do not wear acrylic nails or nail polish to this appointment. Please fill out the enclosed questionnaire and bring it with you to your appointment. This appointment has been made for you at the Pulmonary Department, located in Building 2 North on the First Floor. Important note: the equipment will need to be returned to the Pulmonary Department between 7-8 a.m. the following morning. Someone else may return the equipment for you. (NOTE: if your appointment is on a FRIDAY, the equipment will need to be returned to the Pharmacy in 1 North between 9 a.m. and 9 p.m. on Saturday or Sunday.) If the appointment listed above is not convenient, please call (408) 972-6560 to reschedule your appointment. Thank you. Pulmonary Department/Sleep Lab
