WELCOME TO OUR DEPARTMENT

We are a group of physicians who provide primary care for patients in Skilled Nursing Facilities. Our patients include those who need short term skilled nursing services and rehabilitation, and also long term care residents.

We provide consultative services to hospital physicians on geriatric, pain, hospice, palliative care and rehabilitation issues.

We partner with clinic physicians to assist home health nurses to care for home-bound patients and hospice patients.

We are in the process of establishing an outpatient dementia clinic.

Our mission is to provide high quality, comprehensive care across the continuum.

We value proper medical treatment that emphasizes symptom control. We offer care that is accessible, courteous and customized to the patient and family needs. We promote education and advance planning that anticipates future care needs. We are committed to promoting quality geriatric and palliative care.

HOW TO GET CARE

Since we do not have an office based practice, we only see patients in the Skilled Nursing Facilities or by referrals in other venues.
