Adult Primary Care Medicine is organized into a module system for better continuity of care for patients. We encourage all members to have a personal physician or nurse practitioner. A personal provider works closely with his/her associate providers within the module. This assures that if the primary care provider is not available, one of his/her associates is available for any problems or issues, and information will be conveyed to the personal provider. This also assures that individual medical care will be provided by a small group of providers who will gain familiarity with their patients to provide the best possible care.
SCHEDULING AND CANCELING MEDICINE APPOINTMENTS
- Appointments may be scheduled by calling (408) 362-4791. You can either speak to an appointment clerk, or use our Automated Appointment Services for Primary care.
- Appointments may also be scheduled through the Internet at kponline.org in the Members Only section or clicking on the the Appt/Rx refill tab on the top left of the page.
