Appointments & Prescriptions

Appointments

We see adult and some teenage patients only by referral from other physicians or nurse practitioners. Although we see patients for a number of neurologic symptoms and problems, we emphasize neurologic follow-up for the following conditions: Multiple Sclerosis (see M.S. Society Web Page), Movement Disorders (such as Parkinson's Disease, see National Parkinson's Foundation), Seizure Disorders (see Epilepsy Foundation), and Neuromuscular Diseases (eg, amyotrophic lateral sclerosis, myasthenia gravis, muscular dystrophy). We have close contact with primary care providers to assist in the diagnosis and management of common neurologic disorders, and emergency consultation if needed.

(Note: Please arrive 15 minutes before your scheduled appointment to allow time for registration, blood pressure check and rooming. If you register more than 10 minutes late you may be asked to re-schedule your appointment: this to avoid inconveniencing those patients scheduled after you).

NEW: This appointment is for first visits for new referrals. During this appointment a neurologist will take a careful history and perform a neurologic physical exam. At the end of the appoinment your neurologist will discuss the findings, diagnosis and recommendations for further tests or treatment with you, as well as any follow-up visits that are recommended. Usually our attention will be directed toward neurolgic conditions; we will advise seeking help from your primary care provider for other medical concerns or prescription refills.

RETURN: This is a shorter follow-up appointment for patients who have already seen the same neurologist for the same problem, and is usually reserved for movement disorders, seizure disorders, multiple sclerosis or neuromuscular disorder (some exceptions are made). You may contact Neurology at (408) 972-3100 to arrange a return appointment for one of these conditions.

EMG (Electromyogram) or NCS ( Nerve Conduction Study): These tests are performed at the request of the referring physician to electrically diagnose a nerve or muscular disorder. Comprehensive history, physical exam and discussion are not carried out, since this is a ''test'' appointment. Both tests involve some discomfort, and often access to the upper arms, legs and back are necessary so please wear loose-fitting clothing and PLEASE DO NOT WEAR LOTION OF ANY TYPE.

EEG (Electroencephalogram): This is a test of electrical brain activity, usually ordered to search for evidence of seizures. The test is painless. The head is measured and electrodes are placed on the scalp with a paste. The test usually takes about 60 minutes with the patient lying down, during which time you may be asked to hyperventilate or be shown a flashing light. Sometimes your physician desires EEG recording while asleep: in this case you may be asked to deprive yourself of sleep.

Prescriptions

We encourage our patients to have their medications prescribed and refilled by their primary care provider (even if a neurologist wrote the original prescription). This results in one physician being responsible for all medications to accomplish better management of medicine interactions, side-effects and useage patterns. However, for conditions we regularly follow in Neurology, Multiple Sclerosis, Movement Disorders (such as Parkinson's Disease), Seizure Disorders, and Neuromuscular Diseases (eg, amyotrophic lateral sclerosis, myasthenia gravis, muscular dystrophy) we will often write the prescriptions ourselves. Please do not call Neurology for routine medication refills.

Our department will usually provide emergency 7 day refills of medications . For routine refills, please call the Zero Refill Recorder at least 7 days prior to needing the medication.

  • Press 1 to order a refill or zero refill
  • Enter your daytime telephone number, then press #
  • Select pick up or delivery options, if available.
  • Enter your medical record number
  • Enter your prescription number
  • Then choose the following: Enter more refill numbers; Enter medical record numbers and prescription numbers for a different person;
  • Select payment options

You may order prescription refills online: Go to members.kp.org and click on the Appointments/Rx refills link. You will need the prescription number of the medication you're ordering as well as your credit card (if applicable).

If you have not registered already, it is easy to register online. An activation code will be mailed to you within 5-7 business days.
