Welcome to the Hayward/Fremont Kaiser Permanente Department of Psychiatry. I am Dr. Adam Travis, the Chief of Psychiatry at Hayward and Fremont. Our goal at Kaiser Permanente is be your health plan of choice and deliver high quality medical care to you and your family.

We provide clinic based psychiatric services for adults, teens, and children at both the Union City/Hayward and the Fremont facilities. We also provide drug and alcohol recovery services at the Union City/Hayward site. The home page for each of our clinics is listed below:

We are located at two campuses, at the Union City Medical Offices and at the Fremont Medical Center. You may choose to recieve care at whichever facility is more convienient by calling one of the two telephone numbers above between 9 a.m. and 5 p.m. on weekdays. A mental health professional will collect all the information necessary to schedule an evaluation by one of our providers.

There is always a mental health professional available on call, 24-hours per day, seven days per week for emergencies.
