Welcome to the North Valley Department of Medicine home page. This home page is designed to help you get acquainted with the Department of Medicine and access useful information.
The Department of Medicine provides comprehensive, coordinated primary care, and expert subspecialty consultations for adults 18 years and older. We have a long-standing tradition of excellence in primary care, and serve a culturally and socioeconomically diverse patient population.
The North Valley Department of Medicine, which is organized according to the Adult Primary Care (APC) model of health care, is comprised of six individual campuses located in Sacramento, Roseville, Rancho Cordova, Davis, Folsom and Lincoln; all provide adult primary care services. Each team of highly-skilled professionals includes your primary care physician or nurse practitioner. The APC model, an innovative system of providing individualized medical care in the outpatient setting, consists of physicians and nurse practitioners working in concert with allied health care professionals to provide a multidisciplinary approach to patient care. As primary care providers, we place a strong emphasis on preventive care, health education, and appropriate disease management and medical interventions. Your relationship with your primary care provider is designed to be long term and ongoing, in order to provide continuity of medical care and allow you to make informed decisions about your healthcare needs.
Our six campuses provide 18 APC modules consisting of internists, family practice physicians, nurse practitioners, a clinical health educator, a behavioral medicine specialist, a clinical pharmacist, a registered nurse case manager who assists with the care of patients with complicated medical conditions, and medical assistants. The subspecialty services of Allergy, Cardiology, Hematology/Oncology, Nephrology, Gastroenterology, Pulmonary, Rheumatology, Infectious Diseases, Endocrinology and Travel Medicine are also part of the Department of Medicine and work closely with your primary care provider in a unique group practice. The subspecialty services are accessed through referrals from your primary care provider.
In addition, the Department of Medicine utilizes Chronic Conditions (Disease) Management programs to provide comprehensive care to our members with chronic health conditions. These programs include cardiac artery disease (CAD, asthma, cholesterol, diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), hypertension, an Outreach Program designed specifically for our diabetic and CAD patients, and a Multifit Program which is designed to assist our patients who have had a recent heart attack, heart bypass surgery or angioplasty. These programs are accessed through referrals from your primary care provider.

In the Department of Medicine, we work hard to ensure that every patient sees their personal physician or nurse practitioner for all of their health care needs. If you need an appointment, please call the appointment line for the facility closest to you or use the Online Appointment Center. If you are unable to get an appointment with your personal physician or nurse practitioner, please send a message to him or her when you call. They will make every effort to accommodate you, but if it is not possible, you will be given an appointment with one of their closely aligned colleagues on the same "team" or offered an appointment in our weekend clinic.
Getting Here
Please allow plenty of time for parking and check-in 15 minutes before your appointment time. This will better help your doctor to be on time. We have regular and handicapped parking available on-site for our patients.
Medication Safety
Please always bring a current list of your medications including dosage and frequency. It is for your own safety that we ask to verify this list at every visit. You can pick up a yellow medication card at your next visit on which to keep your list. Most people keep this list in their wallet. This is especially important if you should require medical care outside of Kaiser Permanente.
Weekend Clinic
For those patients that are unable to see their personal physician or have an urgent medical problem, there are two weekend clinics; one in Roseville and one in Sacramento. The weekend clinics are open 8:30 a.m. to 12:30 p.m. To make an appointment, please call the weekend clinic closest to you.
