Welcome to the Forms Intake Department at KP Mountain View!

We provide a variety of clerical services. We will help you process your medical claims and insurance applications, complete various forms, and release and obtain authorized health information.

Our goal is to provide high quality service with great emphasis on patient privacy. The protection of patient confidentiality and health information is and has always been an important value of our department. We will not release any information without the patient’s signed consent.
