Adult Primary Care Medicine is organized into a module system for better continuity of care for patients. We encourage all members to have a personal physician or nurse practitioner. A personal provider works closely with his/her associate providers within the module. This assures that if the primary care provider is not available, one of his/her associates is available for any problems or issues, and information will be conveyed to the personal provider. This also assures that individual medical care will be provided by a small group of providers who will gain familiarity with their patients to provide the best possible care.
Adult Primary Care Medicine utilizes Chronic Conditions (Disease) Management programs to provide comprehensive care to our members with chronic health conditions. These programs include cardiac artery disease (CAD), asthma, cholesterol, diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), hypertension, an Outreach Program designed specifically for our diabetic and CAD patients, and a Multifit Program which is designed to assist our patients who have had a recent heart attack, heart bypass surgery or angioplasty. These programs are accessed through referrals from your primary care provider.
Personalized Care
Kaiser Permanente is committed to Personalized Health Care. If you do not have a Physician or Nurse Practitioner that you consider your primary caregiver, please call Member Outreach at (510) 307-2426, or choose your doctor online.
In the Department of Medicine, we work hard to ensure that every patient sees their personal physician or nurse practitioner for all of their health care needs. If you need an appointment, please call the appointment line for the facility closest to you or use the online Appointment center (click on the Appointments/Rx refills tab at the top of this page). If you are unable to get an appointment with your personal physician or nurse practitioner, please send a message to him or her when you call. They will make every effort to accommodate you, but if not possible, you will be given an appointment with one of their closely aligned colleagues on the same "team" or offered an appointment in our after-hours clinic in Richmond.
Getting Here
Please allow plenty of time for parking and check in 15 minutes before your appointment time. This will better help your doctor to be on time. We have regular and handicapped parking available on-site for our patients.
Medication Safety
Please bring a current list of your medications, including dosage and frequency. It is for your own safety that we ask to verify this list at every visit. You can pick up a yellow medication card at your next visit on which to keep your list. Most people keep this list in their wallet. This is especially important if you should require medical care outside of Kaiser Permanente.
