The Memory Clinic is an interdisciplinary program designed to help provide Santa Clara Kaiser members with a comprehensive assessment of their memory problems and link them with needed resources.

Patients referred for a full dementia work-up are first invited to attend an initial group session. Normal age-related changes and causes of memory problems are discussed during the group visit and resources are shared with patients and caregiver/s.

A one-on-one diagnostic evaluation is scheduled following the group visit. The results of this evaluation are reviewed by the Memory Clinic interdisciplinary team, which includes a geriatrician. Then, a diagnosis and its implications are discussed with the patient and the caregiver/s.

Dementia Case Management: A case manager is introduced to the patient and the caregiver/s, should the diagnosis indicate dementia and there is a need for case management. The case manager acts as a partner to provide education, resources, and psychosocial support in an effort to enhance quality of life for both the patient and the caregiver/s.
