Thank you for choosing Kaiser Permanente as your Health Care Provider.
You may start using any of our services at any of our locations as soon
as your membership becomes effective. If you do not know your effective
date, please contact your employer.

As a member you must request a personal identification (PIN) so that you can take advantage of our Members Only session such as making routine appointments on-line, refilling prescriptions, viewing your labs and much more.
