Kaiser Permanente San Jose Medical Center and Gilroy Medical Offices Member Outreach Department is dedicated in providing every member with a personal physician or nurse practitioner.
Select a personal physician or nurse practitioner:
An important part of your health care experience is building a relationship with your personal physician or nurse practitioner. To that end, we encourage all our patients to have a personal practitioner who practices at a location convenient to your home or office.
Physicians or Nurse Practitioners who practice in our Internal Medicine, Family Practice, Obstetrics/Gynecology and Pediatrics Departments are considered personal practitioners. As members of The Permanente Medical Group, our physicians and nurse practicioners practice together, consulting frequently with each other to offer you the benefit of combined expertise and treatment options. They are assisted by a team of skilled nurses, technicians, psychologists, social workers, physical therapists, and other dedicated professionals who work together to provide you with high-quality medical care.
When you need to see a specialist
Your personal physician or nurse practitioner can refer you to a Kaiser Permanente specialist when he or she feels you need specialty care. Some specialty departments, such as our Mental Health and Psychiatry Departments, do not require a referral.
