Welcome to the home page of the Chronic Conditions Management Program!

Our programs are designed to provide the highest of quality care to members with chronic health conditions. Our goal is to help you succeed in managing your chronic condition so that you can lead a healthy and active life. We offer the following programs:

  • Adult Asthma Care Management
  • Cardiovascular Risk Management
  • Chronic Pain Care Management Program
  • Complex Chronic Conditions Case Management
  • Diabetes Care Management
  • Heart Failure Care Management

Our Care Management Programs are made up of Care Managers, including Registered Nurses, Social Workers, Pharmacists, Respiratory Therapists, Dieticians, Clinical Health Educators, and Program Assistants, specific to the programs. We partner with our patients to develop a personalized care program that is specific for each patient, and together, take proactive roles towards management.

Please note that you will need a referral from your Primary Care Provider to be seen in one of our management programs.
