Chronic Conditions Management - Mission:

We will achieve improved health outcomes, access and service for our members with chronic conditions through a systematic, population-based approach that provides individualized, integrated, evidence-based, cost-effective care which will be progressively integrated across the continuum of health care needs.

CCM Program Descriptions

  • Adult Asthma: The adult asthma population management program seeks to help members with asthma to manage their condition by learning to monitor their symptoms, avoid their triggers, and utilize asthma medications appropriately.

  • Coronary Artery Disease (CAD) Secondary Prevention Program: The CAD program focuses on secondary prevention of atherosclerotic disease through risk factor management and appropriate medication.

  • PHASE: Is a population-based approach to ensure comprehensive secondary prevention of atherosclerosis in patients with any of the following: coronary artery disease, diabetes, chronic kidney disease, stroke/transient ischemic attack, peripheral arterial disease, and abdominal aortic aneurysm.

  • Diabetes: The diabetes population management program encourages members with diabetes to achieve and maintain proper blood glucose levels, manage cardiovascular risk factors with appropriate medications, and have regular retinopathy screening and food exams.

  • Heart Failure Program: This program seeks to increase the quality of life for heart failure patients through proper medication management and diet and exercise education.

  • Chronic Pain Care Management: This program is a behaviorally based group intervention program that targets patients with chronic benign pain syndrome who have been unsuccessfully treated with tradional medical treatments.

  • Complex Chronic Conditions: This program is designed to meet the needs of our frail elderly, high utilizing, and comorbid populations who would most benefit from an intensive case management program.
