Personalized Care

Kaiser Permanente is committed to Personalized Health Care. If you do not have a Physician or Nurse Practitioner that you consider your primary caregiver, please call Member Services at (650) 299-4291 or select one at The Permanente Medical Group Physician Home Pages.

In the Department of Medicine, we work hard to ensure that every patient sees their personal physician or nurse practitioner for all of their health care needs. If you need an appointment, please call (650) 299-2015 or use the online Appointment Center accessed through the Appointments/Rx refills tab at the top of the page.

What If I Am Hospitalized?

Getting Here

As described above the Department of Medicine is located in two buildings, the Cypress Building and the Aspen Building. Please make a note of where your doctor is located before your appointment. Please allow plenty of time for parking and check-in 15 minutes before your appointment time. This will better help your doctor to be on time. The majority of parking is available in the parking garage on Veterans Blvd. near Maple St.

Medication Safety

Please always bring a current list of your medications including dosage and frequency. It is for your own safety that we ask to verify this list at every visit. We are very fortunate to have the technology to view on our computer every prescription you have filled at any Kaiser Permanente pharmacy throughout the Northern California region. However, we want to confirm what you are actually taking in case something has changed since your last visit. You can pick up a yellow medication card at your next visit to keep this list on, and update it as needed. Many people keep this in their wallet. This is especially important if you should require medical care outside of Kaiser Permanente.

If you think you have a MEDICAL OR PSYCHIATRIC EMERGENCY, CALL 911 IMMEDIATELY or go to the nearest hospital. DO NOT attempt to access emergency care through this web site. An emergency medical condition is a medical or psychiatric condition that manifests itself by acute symptoms of sufficient severity (including severe pain) such that you could reasonably expect the absence of immediate medical attention to result in any of the following: serious jeopardy to your health, serious impairment to your bodily functions, or serious dysfunction of any bodily organ or part. An emergency medical condition is also "active labor," which means a labor when there is inadequate time for safe transfer to a Plan hospital (or designated hospital) before delivery or if a transfer poses a threat to the health of the member or unborn child.
