The Redwood City Medical Center offers the following services to support your efforts in managing diabetes. Whether you are newly diagnosed or need a refresher, take advantage of these services designed to help you. No co-pay for most services (depending on coverage), unless indicated by a *.

Diabetes Basics: Learn details regarding the 4 key areas of diabetes management: healthy eating, exercise, medications, and testing your own blood sugar. Bring your glucometer (blood sugar monitor) if you have one. If you do not have a meter, one will be ordered for you. Call for details.

Blood Sugar Log

Diabetes Basics Part 2: Based on your most recent lab results, we will review YOUR risk of developing diabetic complications such as blindness, kidney disease, stroke and heart attack. We will discuss medications to reduce these risks, including benefits and side effects of each therapy. We will also address blood pressure and cholesterol control. At the end of class, a clinical pharmacist will meet with you one-on-one to discuss your medications and answer any questions you may have.

Blood Sugar Log

PHASE for Diabetics: A Clinical Health Educator (CHE) will review your blood sugar diary, discuss 8 ways to Prevent Heart Attacks and Strokes Everyday, and answer your diabetes-related questions. Call for details.

Diabetes Nutrition: Get tips on planning meals, counting carbohydrates, and reading food labels from a Registered Dietitian. Learn new ways to prepare your favorite foods and enjoy eating out.

Living Well with Diabetes: Take an in-depth look at your diabetes self-care skills and advance to the next level in a supportive group setting. General knowledge about carbohydrates and blood sugar monitoring are required. Offered quarterly. Attending all 4 sessions is highly encouraged.

Insulin Start: Learn the benefits of insulin and the proper way to inject insulin as a part of your treatment plan. Call for details.

Savor the Season: This is a quarterly class designed to provide you with season-specific tips to live well with diabetes. Check back for upcoming class dates and times.

Healthier Living with Ongoing Health Conditions: Learn to manage your symptoms, fight fatigue and frustration, make daily tasks easier, and get on with your life. Six 2.5-hour sessions. Call (650) 299-2433 for more information.

Diabetes Help Group: Share your successes and challenges, and learn from other Type 2 diabetics and Guest Speakers. Call (650) 299-2484 for details.

1st Wednesday of each month, 5-6:30 PM

One-on-One Support

Clinical Health Educator (CHE) office visits: Work with a CHE on an ongoing basis to improve your eating habits, increase exercise, stick to your treatment plan, and monitor your own blood sugar. No charge for Kaiser Permanente members.

Registered Dietitian office visits*: Need specific tips on what and how to eat to improve your blood sugar? Ask your doctor or nurse practitioner for a referral.

Nutrition Advice Line: Call (650) 299-3315 to leave a message with a single, specific nutrition-related question. If you have multiple questions, the Registered Dietitian may ask that you get a referral from your MD/NP for an individual appointment. Your call will be returned within 1-3 working days.

Diabetes Care Management Program: Get 1-on-1 care by asking your doctor or nurse practitioner for a referral. Must meet certain criteria to qualify.

- Visit the Health Education Center on the first floor of the Cypress building, next to the pharmacy. Rent or buy DVDs, CDs, and books related to diabetes, healthy eating, physical activity, medications, and testing your own blood sugar. Call (650) 299-2433 for more info.
