Travel Information

Planning to travel? Please fill out a travel clinic assessment form 6 weeks before you leave. Completed forms should be faxed to our travel nurses.

Once the travel nurse receives your form, she will review it and follow up with you by phone within 5-7 days.

For most travelers, filling out and faxing the Travel Clinic Assessment Form should be sufficient.

If any of the items below describe you, please contact your primary care provider about your travel plans. DO NOT FILL OUT THE TRAVEL FORM.

  • Pregnant or trying to become pregnant
  • Have never been seen by any Kaiser provider
  • History of ESRD, organ transplant, HIV/AIDS, leukemia, psychosis, seizures, taking Prednisone or IV/IM chemotherapy

For all travel you should bring the following Travel Medicine Kit:

  • All routine medicines that you or your child takes (e.g. inhalers, skin creams, etc.)
  • A fever reducer (Ibuprofen and/or Tylenol or Acetaminophen)
  • Suntan lotion (SPF 15 or higher, esp. for the tropics)
  • Mosquito repellant (10-25% of DEET is safe for the whole family)
  • Band-aids and simple supplies for cuts, blisters, etc.
  • Pepto-Bismol (for children older than 3 years of age.)
  • Over-the-counter medicines you commonly use, for example Diphenhydramine (=Benadryl), Cough & Cold medicines, an Antibiotic ointment
  • Your yellow immunization card
