Information Guide
Welcome to the Milpitas Occupational Health Center. It is our goal to provide you with prompt, efficient service when you visit our center. This Information Guide is intended to provide you with basic information you will need while receiving care at this facility.
The Milpitas Occupational Health Center is here to provide care for most work related injuries and illnesses as well as preventative services to keep you healthy and safe on the job. Medical conditions not related to your work injury must be cared for by your Primary Care Physician through your Kaiser health plan.
Reporting the Injury: When you visit the doctor, and report that your injury has occurred as a result of your work, our doctor’s are required by law to report the injury/illness to your employer. It is important that you report all work-related injuries to your employer as soon as possible and complete an “Employee’s claim for Workers’ Compensation Benefits” (DWC-1) form, prior to your visit with us. Your employer will provide this form.
Hours of Operation: Business offices 8:00 a.m. to 5:00 p.m., Monday – Friday. Occupational Health Physical Therapy 8:00 a.m. to 6:00 p.m., Tuesday - Friday. Physicians 9:00 a.m. to 6:00 p.m., Monday – Friday. We are closed weekends and Holidays.

Appointments: In order to provide quick access and the shortest wait time possible, you are encouraged to call ahead for an appointment. Patients who drop in without an appointment will be seen by the first available doctor as soon as possible, however, this may result in a prolonged wait. Remember, emergency situations should go to the nearest emergency medical facility.
Test Results: If your doctor has ordered a test, s/he will discuss those results with you at your next visit. If the results require immediate action, your doctor or other Occupational Health Center staff will notify you sooner.
Very Important: Please bring a copy of your “Employee’s claim for Workers’ Compensation Benefits” form to your first appointment. Also, bring any letters, reports, or special forms you have received from your employer or insurance carrier to your appointment. These are important for your claim.
