SUMMER WELL VISIT INFORMATION

Spring 2010

Dear Pediatric Patients and Parents,

The Department of Pediatrics at Kaiser Permanente Santa Rosa is committed to providing your family the best possible medical care all year round. Spring and summer seasons are fast approaching and we look forward to helping you schedule the Well Check and Sports Physical appointments that may be needed for sports programs, summer camps, and school entrance in the fall. Our goal is to complete your health forms and paperwork in a timely manner. Please review our guidelines below regarding Well Check and Sports Physical appointments and how to obtain a Health Certificate.

1. WELL CHECK APPOINTMENTS ARE SCHEDULED AT AGES: 2 months 4 months 6 months 10 months 14-16 months

22 months 3.5 years 5 year

AFTER AGE 5, WELL CHECKS ARE EVERY 2 YEARS UNLESS SPECIFIED (Certain medical plans require annual well checks).

2. IF YOUR CHILD HAS NOT HAD A WELL CHECK IN THE LAST 2 YEARS, WE RECOMMEND THAT YOU SCHEDULE ONE NOW. THERE IS NO REASON TO WAIT UNTIL THIS SUMMER.

3. IF YOUR CHILD HAS HAD AN INJURY OR MAJOR ILLNESS SINCE THE LAST WELL CHECK, PLEASE CHECK WITH YOUR PROVIDER TO SEE IF A WELL CHECK IS NEEDED.

4. IF NO ILLNESS/INJURY SINCE LAST WELL CHECK (< 2 YEARS AGO):

A. CALL (571-3770)OR VISIT THE MEDICAL SECRETARIES IN THE BUSINESS OFFICE IN THE MAIN HOSPITAL TO REQUEST A HEALTH CERTIFICATE FORM. PLEASE BE PREPARED TO SHOW ID. (TURN AROUND TIME IS 3 - 5 WORKING DAYS). B. OR, OUR FRONT OFFICE PEDIATRIC PATIENT SERVICE REPRESENTATIVE CAN FAX YOUR REQUST TO MEDICAL SECRETARY ALONG WITH PHOTO ID AND HIPPA RELEASE. (TURN AROUND TIME IS 3 - 5 WORKING DAYS.)

5. PLEASE MAKE COPIES OF YOUR HEALTH CERTIFICATES/ SPORTS PHYSICAL FORMS FOR YOUR RECORDS AND SCHOOL NEEDS.
