Map and Driving Directions This is as map of the Santa Rosa Kaiser Permanente medical facility with driving directions. Dr. Weitzenberg's Sports Medicine and Physical Medicne clinics are located in the East medical office building (MOB East) on the second floor in Suite 270.
Dr. Todd W. Weitzenberg This is a document that provides information about Dr. Weitzenberg and is helpful to review prior to your clinic appointment to help you become more familiar with Dr.Weitzenberg.
Pain Diagram This is a document you may be asked to complete prior to your visit. It allows you to describe the type of pain you are experiencing. This can be helpful in making a correct diagnosis and monitoring changes in your condition over time.
Spine Questionnaire This is a document you may be asked to complete prior to your visit. It is a 4 page questionnaire that will help facilitate your appointment when you have a complex injury or condition.
P & S Questionnaire This is a document you may be asked to complete if you are having a Worker's Compensation/Industrial Permanent and Stationary report evaluation.
