The Map, E-Mail suggestions, CDC Information

Clinic Map / e-mail / Links

We have four buildings within a 1.1 mile circle. Milpitas is considered a large satellite, and we provide a spectrum of services. You can go from the map to our clinic web site and explore it. Just click on the hot links. You can also access the Santa Clara site by clicking the Kaiser logo on our main page. Enjoy !

E-mail and Links

I hope you enjoyed the entertaining parts; I have some additional links below for medical information.

This website offers helpful medical information to my patients, and parts of the site will change from time to time. Please e-mail me if any of the links on my page do not work, and let me know if you have any suggestions.

Our goal at Kaiser Permanente is to be your health plan of choice and to deliver the quality medical care that our members deserve. Thank you again for choosing us.

If You Would Like to Communicate by E-Mail:

  1. Put your name and medical record number in the subject line.
  2. Please be concise.
  3. Be aware that I usually save e-mails in your chart.
  4. Please do not send any e-mails unrelated to your health care, and please do not put me on '' lists ''.
  5. Expect a reasonable turn around time (about 24-48 hours), particularly if I need to obtain your chart.
  6. INSTEAD OF USING E-MAIL, PLEASE CALL 945-2933:

  • for emergencies and questions that need to be answered promptly

  • if I am away; ( you should get a response to your e-mail that I'm out of the office);

  • for an appointment request.

Disclaimer
If you think you have a MEDICAL OR PSYCHIATRIC EMERGENCY, CALL 911 IMMEDIATELY or go to the nearest hospital. DO NOT attempt to access emergency care through this web site. An emergency medical condition is a medical or psychiatric condition that manifests itself by acute symptoms of sufficient severity (including severe pain) such that you could reasonably expect the absence of immediate medical attention to result in any of the following: serious jeopardy to your health, serious impairment to your bodily functions, or serious dysfunction of any bodily organ or part. An emergency medical condition is also "active labor," which means a labor when there is inadequate time for safe transfer to a Plan hospital (or designated hospital) before delivery or if a transfer poses a threat to the health of the member or unborn child.
