I chose to work with The Permanente Medical Group and Kaiser because I believe that health care for older persons has become too complex to be entrusted to individual providers working in separate (non-integrated) systems. The best way to ensure that care is coordinated and of high quality is to have physicians who are integrated in some basic way. Otherwise too much detail slips through the cracks, which I have found to be a common cause of problems in the elderly. At KP, it's all about integration: computer systems, referrals, ancillary services, scheduling, etc.
My specialty within Geriatrics is post-acute, or transitional care. The days and weeks after hospitalization are precarious ones for frail older persons, and bad outcomes are common during this period. Our post-acute care team is made up of physicians, geriatric nurse practicioners, nursing case managers and social workers. Working in a team that can focus on medical, psychosocial, and emotional issues together is very rewarding.
My other main interet is in health care policy. Obviously folks are living longer, but what effect that has on their overall health is still unclear. The great challenge for geriatricians is to help programs like our's, and Medicare, respond to the intense growth that is currently taking place among the older population. Managing diseases in the elderly is time-consuming and requires teamwork, so creating “geriatric friendly” health care systems is central to keeping older people independent and robust.
My personal hobbies include nonfiction writing, hiking, skiing and golf. But with three young children, my main hobby these days is just playing with them as much as possible before they grow up! My wife and I are avid cooks and 'foodies', I take a multivitamin daily, and we try to do calesthenics and stretching each day on the living room floor (to stay fit, but also to show our kids that exercise is fun, not just important).
